Don’t See Your Plan?
Don’t see your dental plan listed above? We also accept most other PPO plans as an out-of-network provider, and file and process all claims directly with your dental benefit plan company on your behalf. We do not accept HMO plans at this time. For patients without dental benefits, we offer a wonderful in-house membership option. You can learn more about our membership plan here.

Please Note: Payment is due at the time of service.
As a courtesy to you, we will bill your dental benefit plan and track claims. Please let us know of any changes to your plan information. You are responsible for the fees charged by our office, no matter what your plan coverage may be. Most dental benefit plan companies should respond to the claims within four to six weeks. Any remaining cost is your responsibility.
